Submission of Requests

  • Requests must be specific and submitted using this Request Form.
  • For a paper form, please contact us via email or in person.

Inspection of Records

  • Requests are considered made when received by the Official Custodian.
  • Inspections are by appointment during business hours.
  • The Official Custodian will notify applicants if records are not immediately available or not in their custody.
